What is a Performer Work Identification Card?
Any person desiring to perform in an adult entertainment establishment licensed under Jacksonville Municipal Code
Chapter 150 and
Chapter 151 must obtain a Performer Work Identification Card from the Jacksonville Sheriff’s Office.
All performers will be required to obtain and maintain a Performer Work Identification Card on their person or within their custody and control at all times while at the adult entertainment establishment.
A fine not to exceed $500 will be imposed on the adult entertainment establishment for each person who does not have a Performer Work Identification Card.
Restrictions
All performers must be 21-years or older.
Previous Convictions
No permits will be issued to individuals who have been convicted of any Human Trafficking or crimes related to such activity.
If any of these offenses are committed after a Performer Work Identification Card has been issued, the license will be suspended or revoked for the following periods after judicial review:
- Prostitution – 5 years;
- Forcible Felonies – 3 years;
- Narcotics Sales / Trafficking – 3 years;
- Lewd / Obscene Acts – 1 year;
- Possession of Narcotics and / or Narcotic Paraphernalia – 1 year
If a performer is placed on probation for any of the listed offenses, they will not be eligible until their probationary period has been satisfied.
Probation
If an individual is currently on probation for any of the listed offenses, they will not be eligible until their probationary period has been satisfied.
If an individual is currently on probation for any offense other than the offenses listed above, a letter of approval will need to be submitted by their probation officer authorizing their employment at the adult entertainment establishment.

Performer Work Identification Card Application
Please fill out and print the application.
A non-refundable fee of $100 will be collected at the time the application is submitted
. For applications to be processed, applicants must be in possession of a valid form of identification (a state issued driver’s license or state issued identification card). Applications, supporting documentation, and payment must be submitted to Jacksonville Sheriff's Office Headquarters (Police Memorial Building -
501 E. Bay Street, Jacksonville, FL 32202) for processing.
Hours for Application Submissions
- Monday - Friday, 8 a.m. - 5 p.m.
- Closed on holidays and weekends.
Once the paperwork has been processed,
applicants will be contacted at the email address provided to pick-up their ID card. Email correspondence will come from PerformerWorkID@jaxsheriff.org. Any questions about the process can also be directed to this email address.

Materials for Review:
As part of the application process, applicants are required to read Municipal Ordinance 150 and 151 and watch one of the two Human Trafficking videos below. Applicants may choose which of the two Human Trafficking videos to watch.

Lost Cards / Replacement Fee / Renewal Fee
A fee of $35 will be assessed for lost Performer Work Identification Cards.
A renewal fee of $50 will be assessed for Performer Work Identification Cards annually. Please print and fill out the
application again to submit the renewal fee.
